Cancellation Policy
If you need to cancel or reschedule your appointment, please cancel as soon as possible. This will allow another patient to use your appointment time.
We request cancellation 24 hours prior to your scheduled appointment to avoid a cancellation fee of $75.
This fee must be paid in full before other appointments can be scheduled or prescription refills can be provided.
Cancellation charges are not covered by insurance.
No Show Policy
If you fail to show up for your appointment then you will be charged a $50 no show fee. Any new patient who fails to show for their initial visit will not be rescheduled.
Late Arrivals
We respect the time of all our patients and strive to stay on schedule so that you do not have to wait. If you are more than 30 minutes late, we will make every effort to see you but you may have to reschedule.
Payment Information
VISA, MC, AMEX, check, and cash are accepted for in person appointments. You will receive an insurance form for you to submit to your insurance company so that you can be reimbursed.
For televisits, only credit cards are accepted.
Returned check fee is $35.
Lab Fees
When lab tests such as Pap, blood tests, or biopsies are sent to an outside lab, you may receive a bill directly from the lab.
We make every effort to send your specimen to an in-network lab based on your insurance coverage.
Please notify us of lab preferences and we will do our best to accommodate you.
Note: Outside lab fees are separate from fees for your office appointment.
Medical Records Release
Your medical records are available to you or another medical provider. You will need to sign a medical records release authorization that you must submit to our office before we can release your medical records. We will then provide a copy of your records for a nominal labor/copy fee.
Unpaid Balance
• If you have an unpaid balance from a prior visit or encounter, it must be paid before additional care is given, including prescription refills.
